To protect your consumer rights, we need information from you verifying your identity before we can process your request. Please either fax or mail us a letter with:

  1. Your full name
  2. Current address
  3. Previous Address
  4. Phone number
  5. Social Security Number (SSN)
  6. Date of birth (DOB)
  7. Clear copy of your Driver’s License or other form of state issued I.D. Please ensure the copy is legible and can be read easily.
  8. Explanation of your inquiry or dispute.
